Evidently Expands Clinical Data Intelligence from Patient to Population
Clinicians and teams already use Evidently for work that spans many patients, one chart at a time. Over the coming weeks, new tools built on Omni will let them work across all of those patients together, starting with a clinician's own patient list.
PALO ALTO, Calif., October 09, 2026--Today we're announcing the next step in Clinical Data Intelligence. We're bringing the full, real-time picture of the full patient chart to new ways of working with many, from rounding lists to entire populations. Over the coming weeks, we'll be releasing new tools built on top of the Omni foundation, starting with a new way for clinicians to apply Skills and surface insight across the patients they see each day.
Starting with a Full, Real-Time Picture of Patient Complexity
The foundation of Evidently is Omni, an engine that ingests and indexes everything in the patient chart, including data trapped in outside records, scanned documents, and faxes, and creates a real-time picture of a patient's lifetime of complexity. Years of history, reconciled into one clinical picture and mapped to a medical knowledge graph. As new data is added to the chart, it's incorporated in real time. Every day, thousands of clinicians at leading health systems access that full picture of the patient, embedded in the EHR.
Clinicians and Teams Pioneering New Ways to Act on Full Chart Insight
Clinicians and teams have built thousands of Skills to shape Evidently around their work, and much of that work spans many patients. Research teams use it to check patients against a trial's criteria. Population health teams use it to review charts for open care gaps. Care coordinators use it to see what stands between a patient and a safe discharge. Clinicians run the same Skill on patient after patient before a shift or a clinic day. Each of them does it one chart at a time, and the next natural ask has always been the same: "Can I run this on all my patients at once?"
"When we unveiled Omni and Skills in September, the question we heard most was: what comes next? Our customers had already answered it, and we'd already started building it. They were using Evidently for research, population health, and hospital operations, one patient at a time, and asking us to surface insight across their patients, whether it was a 20-patient rounding list or thousands of lives in a value-based care population," said Kai Romero, MD, Head of Clinical Success at Evidently.
From Patient to Population
One of the biggest challenges in patient care and documentation quality is that so much of a patient's story sits where no one can easily see it: the scanned outside record from a decade ago, the fax from a referring physician, the imaging report, the document from another health system's exchange. Most clinical tools never read it. Evidently is the only platform that reads the entire longitudinal patient history before anyone asks, keeps a real-time picture of each patient's complexity, and puts it wherever the health system needs it.
